Request an Appointment Our Appointment Request Process Has Been Updated
We have a new and improved method for appointment requests that is easier than ever. Here’s how it works:
- Please scan the QR code below. You should then see a pop-up screen that includes the phrase “Send Message.” With newer phones, this might appear as an option among other choices.
- You will receive a number in your message app. Click send. You will then receive a response that includes a web link. Click that to initiate the process. Have your personal ID and Insurance card handy. You will not be asked to create an account, nor will you be asked to do any entering of data.
